When D-SNPs operate with exclusively aligned enrollment, states can require those D-SNPs to provide information about the Medicare and Medicaid benefits covered by the D-SNP (and the D-SNP's affiliated Medicaid managed care plan, when applicable) in single, integrated materials to make those materials simpler and easier for D-SNP enrollees to understand. The resources provided on this page share more information about how states can work with d-SNPs to integrate D-SNP materials for current and prospective enrollees.
